Prescriptions & Refills

Please bring your medications or a complete list with you to each appointment. If you need prescription refills, please contact your pharmacy and have them send us a refill request.

Please be sure to give us at least 48 hours notice on all refill requests.

Some chronic or long-term medication refills require an office visit before they may be refilled.

After Hours Emergencies

If you have an urgent matter after hours and need to speak to the physician on call, please dial 512-323-5465. An operator will answer your call and page the physician.

If you experience a life-threatening emergency, PLEASE DIAL 9-1-1 IMMEDIATELY.

Medical Records

A medical release waiver must be signed in order to obtain a copy of your medical records or to have them sent to another health care provider. If you are requesting that your records be sent to another provider we ask that you supply us with their name, address, and phone number.

We require a minimum of 48 hours to process records requests unless it is for an urgent situation.

Forms Completion

We require a 48 hour notice on all forms to be filled out. We will notify you once the form has been completed. There may be a fee required for some forms to be completed.
